Skilled Nursing Care

We provide skilled nursing care within your home. Service includes communicating with physicians for updated care, care plan evaluations, and procedures.

Therapy Services

• Treatment and training/exercise programs designed to restore and preserve function.
• Self care and daily living activities to improve independent functioning.
• Recovery of swallowing, speaking, listening, reading and writing skills. 

Medical Social Worker

Our social workers provide short term counseling with regard to community resources, living arrangement, financial problems, long term planning and services after discharge. 

Home Health Aides

Our HHA provide assistance with personal care and are supervised by licensed nurses, physical, occupational or speech therapists. We provide bathing, skin and oral care, simple range of motion exercises, and linen changes. Visits are between 8:00 AM and 5:00 PM.

We attempt to schedule the same HHA every visit. If a change is necessary, we notify you by phone. When a patient is discharged from home care services, the HHA service also ends. Contact us if you have specific requests and see how we can help you.

Durable Medical Equipment (DME/HME)

DME supplies are covered separately by Medicare and maybe supplied by us or a Medicare certified supplier of your choice.
